Dear Colleagues,
On January 6, 2016, you were sent your electronic 2015 Annual Merit Survey Form. If you didn’t receive the form, please check your Junk mail. We believe that this format will be easier to use and relay the data for the merit review committee. Please follow instruction to complete the form. All merit review documents are due on February 3, 2016.
For Merit Review, you need to complete:
1. 2015 Annual Merit Summary Form (Electronic Form)— The annual merit review process captures a faculty’s teaching, research, and service activities across the previous calendar year (January 1 – December 31, 2015). While the form is virtually the same as last year, please read carefully since necessary adaptations in format were made.
In addition to this form you need to submit to Livetext under Merit Review 2015 area:
1. Updated Curriculum Vita
2. eFRPA (need to complete electronically and follow instructions to generate a PDF or MS-Word document. As you probably know, the eFRPA website is open year-round for you to enter, edit and manage your FRPA-related activities online through the Office of Institutional Research and Effective (OIRE) website at http://www.ucdenver.edu/ir (click “User Login”). Note, you’ll be notified later in March to complete your Faculty Professional Plan for AY 16-17.
