Hello Current Internship Students,
In an effort to gather the most up to date information on your internship sites I need for you to please take a few minutes to complete a short survey of information.
If you have not already completed the Internship Information Survey (as requested by your internship instructor), please click the link below and complete.
You will need the following information (if you have more than one site, please complete a separate survey for each site):
-supervisor name;
-contact information for agency/school;
-supervisor phone;
-names of supervisor’s licenses/credentials;
-has supervisor had supervision training;
-years of supervisor’s post-master’s experience
Thank you for you help in gathering this information, as it is necessary for our CACREP re-accreditation.
